Students who apply must submit the following documents:

• Completed (and signed) application form.
• High School/ GED Diploma or Transcript
• Immunization record for Measles, Mumps, Rubella and DTP/Td/DT

To be considered an in-state resident a student must have resided in Massachusetts for 6 months prior to the date he/she is applying for admission.
